The information we maintain on each burial consists of the name of the deceased, date of burial, age at the time of death, location of their grave and a listing of all others buried within the same lot.  We will mail you a map of the cemetery along with a copy of the grave card showing the above information.

Print the form and mail it to our office at the address below with payment of $10.00 per name requested.
